- Annex A, titled "Agreement on Collection for New Equipment and/or Off-premises Collection," signed by the retailer and collector(s) for any financed equipment.
- Annex B, titled "Data Sharing Agreement with System Managers of the Deposit System for Any Financed Equipment," signed.
- For off-premises equipment installation, include proof of address of the installation site and a photo of the site.
- Send the completed form and quotation to gobeuse@faqdd.qc.ca.
- Receive an acknowledgment email for the submission. Note that this is not a confirmation of acceptance for the financial aid request.
- Step 2: Application Review and Confirmation of Aid Amount
- The application is reviewed for eligibility and compliance with program criteria.
- Receive confirmation of acceptance with details of the granted amount and application identification number.
- Do not proceed with any purchase or installation before receiving this confirmation.
- Step 3: Installation and Submission of Required Documentation
- Once the new equipment is installed, submit the following documentation to gobeuse@faqdd.qc.ca (no postal submissions are accepted):
- The equipment provider's work order.
- Proof of payment by the retailer.
- Evidence of removal of any old equipment (if replacing a machine older than 10 years).
- Photos of the complete installation.
- Your cheque specimen.
- Ensure the equipment is connected online at the time of installation.
- Once the new equipment is installed, submit the following documentation to gobeuse@faqdd.qc.ca (no postal submissions are accepted):
- Step 4: Validation and Disbursement of Financial Aid
- The provided documentation is validated by the FAQDD.
- Financial aid is disbursed following successful validation.
