- Email the completed form to the Foundation by July 15.
- Step 4: Send official transcript
- Arrange for an official transcript with final marks to be sent directly from the issuing institution.
- Ensure the transcript is sent by July 30.
Processing and Agreement
- Applications are reviewed by the OTA Education Foundation Selection Committee.
- Only scholarship winners are notified.
- If selected, the recipient’s funds are released after required identification details are provided.

- The scholarship is administered by the OTA Education Foundation Selection Committee.
- If selected, the recipient must provide a Social Insurance Number for T4A purposes before funds are released.
