Fondation Gracia is a charitable foundation created in 1994 to support the CIUSSS du Nord-de-l’Île-de-Montréal and its surrounding community, particularly in the boroughs of Bordeaux‑Cartierville and Saint‑Laurent. Over 30 years, it has distributed more than 9 million dollars to improve the quality of life of residents in long‑term care and to actively support people receiving palliative care. The foundation mobilizes donors from individuals and businesses and reinvests these funds into concrete projects on the territory.

The foundation structures its financial assistance around several intervention axes. It funds optimal layout and the humanization of living environments in care facilities, such as the reorganization and improvement of common spaces and rooms. It also supports therapeutic and recreational activities that contribute to residents’ well‑being, and finances the acquisition of specialized equipment necessary for care and comfort. A fourth axis focuses on community engagement, backing initiatives aimed at the wider population, when the applicant organization is recognized by the CIUSSS.
Eligible applicants include services and departments of establishments in the CIUSSS du Nord‑de‑l’Île‑de‑Montréal, as well as community organizations in the Bordeaux‑Cartierville–Saint‑Laurent area that provide services to various client groups. Projects must respond to a real need and clearly benefit the target clientele. Priority is given to proposals that demonstrate innovation and have an impact on the entirety of the relevant client group.
General application and accountability framework
Fondation Gracia offers a formal financial assistance process. Managers and administrators of CIUSSS services and recognized community organizations submit their projects using a downloadable application form that must be returned by email before 1 September of the current year. The foundation requires recipients to submit a final report within three months of project completion, describing the results achieved and detailing how the funds were used. Beneficiaries are also asked to share communication materials related to the project and to implement a visibility plan that acknowledges the foundation’s contribution, for example through logos, acknowledgements on signage, plaques on equipment or internal communications.
Governance, values and impact
Guided by values of respect, engagement, integrity, transparency and collaboration, Fondation Gracia manages donor funds with a strong emphasis on accountability. Its publications, including newsletters and financial statements, present its activities and the use of resources. The "Nos réalisations" section highlights funded initiatives such as adapted activities, improvements to living environments, tablets introduced in CHSLD and dedicated spaces created in memory of donors. Through this portfolio of interventions, the foundation plays a significant role in enhancing everyday life within long‑term care and palliative care settings in northern Montréal.
