How to apply
- Step 1: Meet with the ODSP caseworker
- Confirm eligibility for the hearing aid benefit.
- Complete the recipient section of the authorization form.
- Step 2: Visit a registered service provider
- Go to a provider registered with ADP.
- Have the provider complete the remaining application details.
- Step 3: Sign and submit the form
- Sign the authorization form after the device or service is provided.
- The provider sends the completed form, ADP form, and supporting documents to the local office.
- Step 4: Exceptional circumstances, if needed
- Use the Exceptional Circumstances form for items outside the Schedule or requiring special approval.
- Send the request to the Ministry for review.
Processing and Agreement
- Applications are reviewed by the local ODSP office and, for exceptional circumstances, by the Ministry.
- The Ministry may request additional information or a second estimate if needed.
- Applicants and service providers are notified of the decision within 30 business days for exceptional-circumstances requests.
- If approved, payment is processed after the required authorization forms are submitted.
- Decisions may be subject to internal review and appeal.
