Eligibility

Who is eligible?

  • Children 17 years old and under in Ontario.
  • Children receiving Temporary Care Assistance or Assistance for Children with Severe Disabilities are automatically enrolled.
  • Children or their family receiving Ontario Works or Ontario Disability Support Program are automatically enrolled.

Who is not eligible

  • Children who turn 18 during the benefit year are no longer eligible.

Eligible expenses

  • Check-ups, cleaning, fillings, x-rays, and scaling.
  • Tooth extraction.
  • Urgent or emergency dental care, including treatment of a child’s toothache or tooth pain.

Ineligible Costs and Activities

  • Cosmetic dentistry, including teeth whitening and braces.

How to apply

  • Enrol online or by mail.
  • To enrol online, complete the online application.
  • To enrol by mail, download, print and complete the Healthy Smiles Ontario General Application, then mail it to Healthy Smiles Ontario, 33 King Street West, PO Box 645, Oshawa, Ontario, L1H 8X1.
  • If you receive Ontario Works and live in a First Nations community, download, print and complete the Ontario Works First Nations Verification Form, then mail it to Healthy Smiles Ontario, Station P, PO Box 2286, Toronto, Ontario, M5S 3J8.

Processing and Agreement

  • Once enrolment is confirmed, a Healthy Smiles Ontario dental card is mailed.
  • The dental card must be presented to the dental provider at each visit to receive services under the program.
