Processing and Agreement

  • If a file is incomplete, AQLPH may request additional information.

  • The CAL is issued by AQLPH with support from regional disability leisure bodies.
  • The card is valid for 5 years and renewal is not automatic.
  • If the card is lost, stolen, or not received, a replacement can be issued free of charge.
  • For in-person assistance, an appointment is required.
