Overview

Provides up to $3,500 to support community wellness initiatives. Funds local health promotion projects aligned with Community Health Board priorities and population health approaches.

Activities funded

  • Local community projects that improve health and wellness using a population health approach
  • Health promotion initiatives aligned with Community Health Board (CHB) priorities in the current community health plan
  • Projects implementing recommendations from the CHB Community Health Plan
  • Community initiatives addressing Social Determinants of Health (e.g., income, education, social supports, early childhood development)
  • Partnership-based wellness initiatives that support healthier communities
Examples of admissible projects:

Eligibility

  • The applicant must be a non-profit group.
  • The project must aim to improve health in the applicant’s community.
  • The project must address health priorities identified by Community Health Boards (CHBs) in their current community health plan.
  • The project must use a health promotion or population health approach.
  • The project should address Social Determinants of Health.

  • Contact your local CHB Coordinator to confirm the project meets the application guidelines.
  • Identify your local CHB Coordinator using the “Find your CHB” tool on the Community Health Boards website.
