Eligibility
- The applicant must be a non-profit group.
- The project must aim to improve health in the applicant’s community.
- The project must address health priorities identified by Community Health Boards (CHBs) in their current community health plan.
- The project must use a health promotion or population health approach.
- The project should address Social Determinants of Health.
