Provides up to $3,500 to support community wellness initiatives. Funds local health promotion projects aligned with Community Health Board priorities and population health approaches.
Activities funded
- Local community projects that improve health and wellness using a population health approach
- Health promotion initiatives aligned with Community Health Board (CHB) priorities in the current community health plan
- Projects implementing recommendations from the CHB Community Health Plan
- Community initiatives addressing Social Determinants of Health (e.g., income, education, social supports, early childhood development)
- Partnership-based wellness initiatives that support healthier communities

- The applicant must be a non-profit group.
- The project must aim to improve health in the applicant’s community.
- The project must address health priorities identified by Community Health Boards (CHBs) in their current community health plan.
- The project must use a health promotion or population health approach.
- The project should address Social Determinants of Health.

- Identify your local CHB Coordinator using the “Find your CHB” tool on the Community Health Boards website.
