• Completed Trillium Drug Program application form
• Household income documents, if CRA income cannot be verified
• CRA consent signatures for household members aged 18 and over
• Official pharmacy prescription receipts or PMERs for reimbursement
• Private insurance information, if applicable

Who is eligible?

• Ontario residents
• People aged 25 to 64
• People aged 24 and under with a private insurance plan

Who is not eligible

• People already covered by Ontario Drug Benefit
• People with insurance that pays 100% of drug costs
• Residents outside Ontario

Eligible expenses

• Eligible prescription drug costs covered under the Ontario Drug Benefit program.
• Some drugs covered through the Exceptional Access Program.
• Official prescription receipts for reimbursement of out-of-pocket covered drugs.

Ineligible Costs and Activities

• Prescription drugs bought outside Ontario.
• Drugs not listed on the Ontario Drug Benefit formulary and not approved through Exceptional Access.
• Cash register receipts, credit card receipts, and clinic or doctor invoices.

Eligible geographic areas

• Ontario

• Applications are reviewed after submission through the online portal or by mail.
• The ministry confirms household income and supporting information before enrolment is completed.
• A confirmation letter is issued after enrolment, including the yearly deductible.
• Enrolment renews automatically each year if requirements are met.
• Changes to income, insurance, or household membership may trigger reassessment or an updated review.

• The program year runs from August 1 to July 31.
• Enrollment is renewed automatically each year if tax information is filed on time.
• Covered drugs must generally be filled in an Ontario pharmacy.
• Receipts and supporting documents have a filing deadline after the program year ends.
