Participants can receive up to approximately $50,000 per team to implement or strengthen a promising practice. Funding supports improvements that advance equitable access to safe, high-quality team-based primary care closer to home, including culturally safe care.

- The applicant must be a group prioritizing team-based primary care in northern, rural, or remote communities.
- The project must focus on implementing or strengthening a promising practice in team-based primary care.
- The initiative must aim to improve equitable access to safe, high-quality primary care closer to home (including culturally safe care).
- The applicant can be at any stage of a quality improvement journey (from need identification to implementation).

- HEC planned to notify applicants of the outcome by the end of October 2023.
