Eligibility

  • The applicant must be a group prioritizing team-based primary care in northern, rural, or remote communities.
  • The project must focus on implementing or strengthening a promising practice in team-based primary care.
  • The initiative must aim to improve equitable access to safe, high-quality primary care closer to home (including culturally safe care).
  • The applicant can be at any stage of a quality improvement journey (from need identification to implementation).

  • Up to 20 teams across Canada will be supported.
  • Groups at any stage of a quality improvement journey are encouraged to apply (from identifying a need to implementation).
  • HEC planned to notify applicants of the outcome by the end of October 2023.
  • Program activities were planned to run from November 2023 to September 2024.
