• Patients receiving care at Centre hospitalier De Lanaudière in Joliette
• Women and men undergoing compatible short-infusion chemotherapy

Eligible expenses

• Manual cooling cap rental for the full duration of treatment
• Dry ice and associated delivery

Ineligible Costs and Activities

• Purchase of a cooler if all five 56.8-liter wheeled coolers are already in circulation
• Delivery fees are the beneficiary’s responsibility, except the delivery associated with dry ice, which is included in the program
• No retroactive reimbursement for orders placed before the program’s start date
• Rental fees are charged if the equipment is returned after five business days

  • Submit the form for eligibility review.
• Step 2: Eligibility review
  • A representative from the Centre intégré de santé et de services sociaux de Lanaudière reviews the request.
  • Eligibility is confirmed within 24 to 36 business hours.
• Step 3: Receive ordering instructions
  • If approved, you receive an email with the ordering process.
  • A code is provided to order the caps on the manufacturer’s website.
  • Dry ice must be ordered by phone using the account number provided.
