- Constitutive documents and financial statements of the company and related companies for the 12 months before the application
- Organizational charts and ownership details for the company and related music businesses, including the internal organizational chart
- Lists of directors, members, partners or shareholders, plus resolutions, bylaws and agreements affecting ownership or control
- Board resolution authorizing the application and designating an authorized signatory, and show contracts if the artist is linked to a company supported under Enveloppe de financement global

How to apply
- Submit the Project tab of the form Demandeur – Artiste, Pro. ou Music. – MP electronically to inscription@musicaction.ca
- The form must be completed and signed
Processing and Agreement
- Accepted projects are not systematically verified; targeted and random selections are made to reach a minimum verification rate of 30% of Musicaction commitments.
- The first sample of supporting documents generally corresponds to 40% of the lesser of the submitted expenses or the expenses required to justify the commitment.
- After the completion analysis, the applicant must submit the selected invoices and proofs of payment; unsupported amounts are deducted and additional documents may be requested, up to 100% of the submitted expenses.
- The second payment is issued after the completion file is analyzed.
- Original invoices and proofs of payment must be kept for five years after the end of the project and may be requested at any time.
