Ineligible Costs and Activities

• Prescription drugs purchased outside Ontario
• Drugs not listed on the formulary or approved through EAP
• Nutrition products for weight loss, body building, meal replacement, or food allergies
• Items such as cash register receipts or credit card slips

Eligible geographic areas

• Ontario

• Income verification against program thresholds.

• For some benefits, covered drug status and valid special authorization.

• Step 1: Complete the online application
  • Fill out the Seniors Co-Payment Program application on the Ontario Drug Benefit forms website.
  • Use the paper form if online completion is not possible.
• Step 2: Provide required information
  • Include your Ontario Health card information.
  • Include spouse information and signature, if applicable.
  • Provide CRA consent or income proof documents if requested.
• Step 3: Submit the application
  • Submit the form online through the Ontario Drug Benefit forms portal.
  • Mail the application if using paper submission.
• Step 4: Submit reimbursement receipts, if needed
  • Upload receipts online through the program portal or mail them to the program address.
  • Include official pharmacy receipts or PMER documentation.
