Selection criteria

How to apply

- Obtain proof of active hospital privileges.
- Include a hospital official letter where required.
- Step 3: Submit the application
- Send the application by email or fax to the ministry-designated account.
- Do not mail or courier the documents.
Processing and Agreement
- Application is reviewed for eligibility.
- The applicant receives an approval or denial letter.
- Approved funding is paid through OHIP claims payment system.
- Processing takes about 8 to 12 weeks after all required information is received.
