Eligibility
Who is eligible?
- Any person with a disability and limited in the performance of daily activities.
- The person must be a Canadian citizen or permanent resident.
Who is not eligible
- People who are already eligible for financial assistance for home adaptation under, among others, the SAAQ or CNESST insurance plans.
How to apply
- Obtain the Program Registration form from a CLSC or the SHQ.
- Complete the form and send it to the SHQ.
Processing and Agreement
- The SHQ forwards the application to the health establishment responsible for the occupational therapist's report and to the partner MRC.
- The MRC prepares the file, establishes the list of eligible work, and determines the amount of financial assistance required.
- The owner must wait for authorization from the MRC; the certificate of eligibility constitutes this authorization.
- The work must be completed within twelve months following the issuance of the certificate.
- The financial assistance is paid in full to the owner at the end of the work, after verification by the MRC.
