How to apply
- Step 1: Complete the application form
- Fill in the employee identification, employment, and contact information.
- Sign and date the request.
- Step 2: Attach supporting documents
- Add a cheque specimen if your banking details changed or you want deposit to another account.
- Attach proof of right to receive the payment, if applicable.
- Step 3: Submit the request
- Send the form to the HR department of your last employer.
- Use the mention “Forfaitaire étudiant” on the submission.
- Step 4: Use an accepted delivery method
- Submit by email or by mail to the addresses provided by the MIFI.
Processing and Agreement
- The application is reviewed by the HR department of the applicant’s last employer.
- If approved, payment is issued by direct deposit or by cheque depending on the case.
- Supporting documents may be required for heirs or authorized representatives.
Additional information
- The amount is calculated at $1 for each paid hour during the eligible period.
- Students and interns are treated differently under the measure: interns are not covered.
- Only work performed during the school year is covered; summer-only work is excluded.
- The application had to be sent before December 20, 2023.
