Documents Needed
- Documentary evidence establishing the identity of the deceased first responder, the identity of the applicant, and the relationship between them
- A court-certified copy of the last will and testament, letters of administration or other similar documentation, as may be required if there is no eligible beneficiary
- A duly completed application form
- An attestation from an authority authorized to represent the employer organization
- Medical records or other records, a certified true copy of the death certificate, and other documents or evidence as may be required to confirm eligibility

Processing and Agreement
- The Memorial Grant may be payable following submission of a properly completed application and a determination that the applicant is an eligible beneficiary and the death of the first responder is attributable to, and resulting from, the performance of duties
- The absence of a signed bilateral agreement will not delay a payment to an eligible beneficiary

- OXARO has been contracted by Public Safety Canada to administer the Memorial Grant Program application process.
- The Memorial Grant is a non-economic benefit and does not compensate beneficiaries of first responders for monetary loss (income replacement) or serve as life insurance, but rather, is in recognition of their service and sacrifice.
- The Program will comply with all applicable requirements stipulated in the Official Languages Act and related regulations, as well as federal policies in this regard, including Chapter 4-1 of the Official Languages Policy.
