Provides up to $7,500 in operational support for community-based programs that directly support family caregivers. Eligible activities include educational programming, transportation, meal assistance, respite care, emotional/financial/community support, and healthcare navigation.

Define funded activities

  • Submit your application during the intake period (September 5 to October 17, 2025)
  • Ensure it is sent by 11:59 PM Mountain Time on the deadline date
