Eligibility
- Apply as a group of at least two organizations working together.
- Project focuses on improving access to palliative approaches to care for people experiencing homelessness or vulnerable housing.
- Be prepared to proceed to implementation during the collaborative.
- Have meaningful partnerships with the people who stand to benefit from the services and programming.

- Cohort selection will reflect a mix of settings, geographies and populations.
How to apply
Step 1: Build a community partnership
- Form a group of at least two organizations to apply together.
- Confirm the joint focus on improving access to palliative approaches to care for people experiencing homelessness or vulnerable housing.
- Ensure the community group is prepared to proceed to implementation during the collaborative.
- Demonstrate meaningful partnerships with people who stand to benefit from the services and programming.
- If successful, work with HEC or complete the formal application independently.
- Submit the formal application by October 25, 2023.
- Submit the application in English or French.
- Receive notification of the outcome by the end of January 2024.

- If successful, applicants must complete and submit a formal application by October 25, 2023 (with support from HEC or independently).
- Applications may be submitted in English or French.
- HEC will notify applicants of the outcome by the end of January 2024.
