Overview

This initiative provides up to $100,000 over two years to improve access to palliative approaches to care for people experiencing homelessness or vulnerable housing. Eligible activities focus on implementation-ready, community-based work delivered by at least two partner organizations.

Activities funded

  • Community implementation projects that improve access to palliative approaches to care for people experiencing homelessness or vulnerable housing.
  • Collaborative initiatives delivered jointly by partner organizations (e.g., social programs, shelters/housing facilities, community health centres, hospices).

  • Apply as a group of at least two organizations working together.

  • Cohort selection will reflect a mix of settings, geographies and populations.

How to apply

Step 1: Build a community partnership
  • Form a group of at least two organizations to apply together.
  • Confirm the joint focus on improving access to palliative approaches to care for people experiencing homelessness or vulnerable housing.
Step 2: Prepare for implementation and partnership engagement
  • Ensure the community group is prepared to proceed to implementation during the collaborative.
  • Demonstrate meaningful partnerships with people who stand to benefit from the services and programming.
Step 3: Complete the formal application
  • If successful, work with HEC or complete the formal application independently.
  • Submit the formal application by October 25, 2023.
  • Submit the application in English or French.
Step 4: Await decision
  • Receive notification of the outcome by the end of January 2024.

  • If successful, applicants must complete and submit a formal application by October 25, 2023 (with support from HEC or independently).
  • Applications may be submitted in English or French.
  • HEC will notify applicants of the outcome by the end of January 2024.
