- Families referred by parents, guardians, professionals, hospitals, rehabilitation centres, long-term care facilities, or wish-granting organizations

How to apply
- Step 1: Complete the request form
- Fill in the child’s and parent or guardian’s information.
- Include the diagnosis details.
- Step 2: Attach supporting letter
- Add the confirmation letter from a healthcare professional.
- Step 3: Submit the request by email
- Send the form and letter to jguindon@espaceespoir.org.
- Step 4: Wait for follow-up
- If the request is being considered, Julie Guindon will contact the family.
