Dental and denture fee guides updated
The Dental & Denture Services page now shows new service and fee guides effective July 1, 2026 for both the Discretionary Adult Emergency Dental Treatment Plan and the Denture Program, with the page date modified to July 2, 2026. It also now states that Ontario Works is the second payor when a client has Canadian Dental Care Plan coverage, and that the Sun Life Explanation of Benefits must be submitted with the Dental Claim form.

Who is eligible?
- Ontario Works Adults, age 18 years and over, resident of the city of Hamilton
- Ontario Disability Support Program Dependent Adults, age 18 years and over, not covered on the ODSP Dental Card, resident of the city of Hamilton
Who is not eligible
- Ontario Works children (0-17 years), including children whose guardian receives Temporary Care Assistance under Ontario Works, are not covered under the Adult Discretionary Dental Plan.
- Ontario Disability Support Program recipients, their spouses and dependent children (0-17 years), and children whose parent(s) receive Assistance for Children with Severe Disability (ACSD), are not covered under the Adult Discretionary Dental Plan.
Eligible expenses
- Examinations and radiographs, including intraoral and panoramic
- Restorative services, including amalgam and tooth coloured/plastic restorations and retentive pins
- Endodontic services for permanent upper/lower anterior teeth only, including pulpotomy, pulpectomy, and root canals
- Periodontal abscess or pericoronitis treatment, oral and maxillofacial surgery, surgical incision and drainage, avulsed tooth treatment, and nitrous oxide
- Adult denture services
Ineligible Costs and Activities
- Cleaning and scaling
- Root canals on back teeth
- Crowns and bridges
- Orthodontics of any kind, including services related to space maintenance and crowding
- Night guards, sedation, and treatment of primary (baby) teeth

How to apply
- For full mouth clearance, submit a preauthorization request by mail, fax, or email at specialsupportsinvoices@hamilton.ca.
- Include an estimate showing all procedure codes and fees.
- Include a written explanation regarding treatment plan for dentures.
- Submit original claim forms by email or mail.
- Send claims as treatment occurs; multiple appointment procedures such as root canals should be submitted on completion of treatment.
- If a patient is missing the dental card or approval letter, the patient must contact Ontario Works prior to receiving treatment to request a replacement be faxed or emailed to the dental practitioner.
Processing and Agreement
- When predetermination is approved, the Special Supports Program will mail the applicant an approval letter to take to the denturist or dentist.
- Approved payment information is sent to the dentist by password protected email, with an option to receive it by regular mail.
- If payment is not received within 45 business days, do not submit duplicate claim forms; call to verify whether the original claim was paid or received.
- Each year, service providers receive notice of the last date claims will be accepted and honoured for payment.
- After that date, no claims from the previous year will be accepted or honoured for payment.
