Eligibility

Who is eligible?

  • People with physical or functional limitations or health conditions that are unable to use the regular HSR bus.
  • City of Hamilton residents may access the Taxi Scrip Program with ATS.

Eligible expenses

  • Dental examinations, x-rays, fillings, restorations, extractions, and selected root canal treatment
  • Emergency dental treatment within the fee guide limits
  • Adult denture services

How to apply

  • Print the applicable ATS application form.
  • Complete Part 1A to 1G.
  • Have Part 2A and 2B completed by a qualified health care professional.
  • Complete Part 3 only if authorization is needed to share information with other transit properties.
  • Submit the application by mail, fax, or email.

Processing and Agreement

  • ATS will determine eligibility within up to fourteen (14) calendar days.
  • Applicants may be asked to complete a third-party functional assessment.
  • If eligible, passengers may receive unconditional, conditional, or temporary eligibility.
  • If a decision is appealed, the Eligibility Appeal Panel will review the application information and any additional information provided.
  • The panel’s decision is final.

Additional information

  • Healthy Smiles Ontario provides free preventive, routine, and emergency dental services for children and youth 17 and under from low-income households.
  • For some programs, dental or denturist offices can send predeterminations to the Special Supports Program by fax or mail.
  • Approved predeterminations are followed by an approval letter to take to the provider.
  • Ontario Works is the second payor if the client has Canadian Dental Care Plan coverage.
