How to apply
- Print the applicable ATS application form.
- Complete Part 1A to 1G.
- Have Part 2A and 2B completed by a qualified health care professional.
- Complete Part 3 only if authorization is needed to share information with other transit properties.
- Submit the application by mail, fax, or email.
Processing and Agreement
- ATS will determine eligibility within up to fourteen (14) calendar days.
- Applicants may be asked to complete a third-party functional assessment.
- If eligible, passengers may receive unconditional, conditional, or temporary eligibility.
- If a decision is appealed, the Eligibility Appeal Panel will review the application information and any additional information provided.
- The panel’s decision is final.
Additional information
- Healthy Smiles Ontario provides free preventive, routine, and emergency dental services for children and youth 17 and under from low-income households.
- For some programs, dental or denturist offices can send predeterminations to the Special Supports Program by fax or mail.
- Approved predeterminations are followed by an approval letter to take to the provider.
- Ontario Works is the second payor if the client has Canadian Dental Care Plan coverage.
