Activities funded

- DFF Application for Support Form
- Copies of the Notice of Assessment from the Canada Revenue Agency for the past two years for each legal guardian
- A void cheque, which is preferred, or completed banking information
- A member of the Transplant Team must complete and sign the medical information section of the application

Who is eligible?
- Canadian families with children under 19 who need a life-saving organ transplant.
Eligible expenses
- Travel for medical appointments, including car-related expenses based on annual mileage guidelines provided by the Canada Revenue Agency and flights.
- Accommodations.
- Food.
- Childcare support.
- Other financial support, including rental or mortgage support, utilities, clothing, toiletries, other regular payments that cannot be renegotiated or put on hold, and funeral expenses up to $6,000.
Ineligible Costs and Activities
- Medical expenses, including prescription medications, medical care and equipment.
- Potential donor(s) in living donor transplant assessments.
- Car rentals.
How to apply
- Speak with your transplant social worker to receive a DFF Application for Support.
- Complete the DFF Application for Support Form.
- Have a member of the Transplant Team complete and sign the medical information section.
- Submit the application through a family’s social worker or a resource navigator at their home hospital or transplant centre.
Processing and Agreement
- Each application is reviewed and assessed based on the information provided by the family.
- Approvals can take up to a week once the application is complete.
- Original receipts must be sent on a regular and timely basis.
- Only itemized receipts are accepted unless otherwise arranged with the DFF office.
