  • Complete our online application form and make sure all fields are completed before you submit.
  • Submit the completed form through the link provided below.

Processing and Agreement

  • Once the application is submitted, you will receive an email confirming it has been received.
  • Requests are reviewed from January 1 to October 30 each year.
  • Applications submitted in November or December are reviewed in January of the following year.
  • At least 60 days are required to review a request and respond with a funding decision; larger requests can take up to six months.
  • Applicants are notified of the outcome as soon as a decision is made.
