- Complete our online application form and make sure all fields are completed before you submit.
- Submit the completed form through the link provided below.
Processing and Agreement
- Once the application is submitted, you will receive an email confirming it has been received.
- Requests are reviewed from January 1 to October 30 each year.
- Applications submitted in November or December are reviewed in January of the following year.
- At least 60 days are required to review a request and respond with a funding decision; larger requests can take up to six months.
- Applicants are notified of the outcome as soon as a decision is made.
