Eligibility

Who is eligible?

  • Parent or legal guardian of the child or children involved in the incident

Who is not eligible

  • People who are receiving Employment Insurance benefits
  • People who are receiving Quebec Parental Insurance Plan benefits
  • People who have been charged with committing a probable Criminal Code offence that led to the death or disappearance of the child

How to apply

  • Prepare the required information, including your SIN, addresses, number of weeks requested, banking details, employer information, and police incident report number.
  • Gather the required original completed and signed forms.
  • Apply online if you can.
  • If you cannot apply online, print and complete the application form, then mail it or drop it off at a Service Canada Centre.
  • Upload your required documents when you submit the online application, or upload them separately later if they are not ready.
  • If applying by mail or in person, mail or submit the required documents with the application form.

Processing and Agreement

  • Once your application is complete, the program may contact you for more information.
  • The program may contact your employer to confirm or update the information provided.
  • The program may contact the law enforcement agency to confirm or update the information provided.
  • If the application is approved and all required documents were submitted, payments begin immediately.
  • If banking information was provided, payments are made by direct deposit; otherwise, a cheque is sent by mail.
  • At the end of the year, you receive a T4A slip for income tax purposes, and Quebec residents also receive an RL-1 slip.
