Eligibility

Who is eligible?

Selection criteria

  • Applicants who are receiving end-of-life palliative care and/or have been diagnosed with a terminal illness are prioritized.

How to apply

  • Apply online for a Disability Income Assistance application.
  • If you cannot apply online, complete the Disability Income Assistance application form.
  • Submit your Disability Assistance Medical Report as part of your online DIA application or ask your health professional to submit it by fax or mail.
  • Submit your DIA Application and supporting documents online, by fax, or by mail.

Processing and Agreement

  • An AISH and ADAP staff member may request additional information if you have not provided all of the personal, financial or medical information required.
  • When AISH and ADAP finds an overpayment, you will be sent a Notice of Overpayment.
  • If you disagree with the Notice of Overpayment, you have 30 days from the date you get it to make a written request or file a written appeal.

Additional information

  • If you receive AISH, you must inform the AISH program of the approval or denial of your Canada Disability Benefit and/or Disability Tax Credit application.
  • You are responsible for reporting any changes in your situation to an AISH or ADAP worker as soon as possible.
  • Payments are made via direct deposit.
