How to apply
- Sign up for My Family Services.
- Complete the online application in My Family Services, or download and complete the paper Application form (CF2900) if you cannot apply online.
- Have your child care provider complete and sign their sections of the Child Care Arrangement form (CF2798).
- Upload all required supporting documents with your application.
- Submit the application online through My Family Services or by mail or fax.
Processing and Agreement
- Once your application is approved, both the parent or guardian and the child care provider receive a benefit plan with the benefit amount and the start and end dates.
- In most cases, the child care provider receives payment after submitting a monthly claim form.
- If child care is provided in the child’s home, the parent or guardian submits the claim form and pays the provider.
- You may ask the ministry to reconsider a decision, and the Request for Reconsideration form must be submitted within 20 business days of notification.
- To appeal a reconsideration decision, submit the Notice of Appeal form within 20 business days of receiving the decision.
