How to apply

  • Step 1: Complete tax return calculations
    • Enter eligible medical expenses on federal line 33099 to 33200.
    • Enter medical expenses on Ontario Form ON428, lines 58689 to 58769.
  • Step 2: Claim the credit on Form ON479
    • Use Form ON479, Ontario Credits, to claim the tax credit.
    • Complete lines 4 to 14 of Form ON479.
  • Step 3: Keep records
    • Keep receipts for all eligible expenses claimed.
